If you are not seen in the Center for Sleep for 2 years, your chart will be closed. (Call 20, extension 52124, for the Medical Review section.) The Center for Sleep Disorders will not mail or fax the form for patients. Patients should call the Bureau of Motor Vehicles to make sure that their forms have been received once signed by the Center for Sleep Disorders. Patients will be responsible for making sure that the Bureau of Motor Vehicles receives their signed form. If you feel that your form needs to be filled out urgently, you may discuss with your primary care provider. Urgent appts cannot be accommodated due to high volume of appointments for clinical issues. Modems do not always function and therefore your data may not be available if you solely rely on that. It is recommended that you bring your machine’s data card to all of your follow-up appointment– even if your machine has a modem. It is recommended that patients do this at least 2 weeks before their appointment at the Center for Sleep Disorders. If compliance data is not available at the time of the office visit appointment, then the BMV form cannot be filled out. Patients must obtain a physical copy of their compliance data from their Durable Medical Equipment company prior to their appointment. It is the patients’ responsibility to keep track of when their form is due. Patients should make sure their forms are reviewed at least 60 days before their license is set to expire. Please note that due to high volume, driver’s license forms for Sleep Apnea for the Bureau of Motor Vehicles (BMV) are now reviewed by in-person appointment only.
